- A copy of the applicant's inoculation records.
- A copy of the child’s certificate of birth.
- Parents or legal guardians Identification documents
- Study visa
- A consent letter
- Duly filled and signed application forms
- A non-refundable application fee proof of payment
- passports size photographs
- parent or guardian’s valid passport
- a copy of the parent's residence permit (if residing in chad)
- Proof of residence in chad
